PharmAssist Concierge Pharmaceutical Services, PLLC (DBA PharmAssist On Demand) is a community-based pharmaceutical care delivery organization that provides quality service across the spectrum of patient points of care.
PharmAssist On Demand operates under the leadership of Founder and pharmacist Halle Harrison. Halle Harrison attended Hampton University School of Pharmacy in Hampton, Virginia. Throughout her matriculation, she interned with Kaiser Permanente and CVS Health to sharpen her clinical and analytical skills. Upon graduation in 2017, Halle Harrison worked as a Staff Pharmacist with CVS Health prior to moving into her current role as a Clinical Pharmacist at QuVa Pharma, Inc. During her time as a Staff Pharmacist with CVS Health, Halle noticed the overarching lack of patient counseling and its role in contributing to the widening disparities in care among patients. Thus, the idea came into fruition to create a pharmaceutical care organization under a concierge model to improve patient access to care and reduce disparities.
PharmAssist on Demand currently provides care to men and women 60+ within the Houston metropolitan area and is responsible for managing drug therapy. With a goal of increasing patient access to high quality, affordable healthcare services and enhancing patient health outcomes, PharmAssist On Demand uses a service delivery model that is patient-centric – focusing on physical interactions with patients in a variety of settings, including physicians’ offices, ambulatory and outpatient centers and personal residences, as well as using technology.
At PharmAssist On Demand, the pharmacists bring the pharmacy to the patient. We act as liaisons between patients and medical personnel, thereby reducing gaps in care and healthcare disparities and ensuring that patients get timely access to critical healthcare services.
PharmAssist On Demand professionals meet with patients on a weekly basis to review medication therapies (and, if necessary, reconcile them), monitor patient adherence and make lifestyle modification recommendations. As a convenience, patients can schedule deliveries for medications and choose between in-home or clinic-based appointments via the patient portal or telephone. These streamlined operations allow providers to reach patients from varied backgrounds and reduce disparities in care so that more patients can have timely access to healthcare services.
